Organization of psychological assistance to the populationin the service of emergency operative calls "112" in the Volgograd Region: experience of the first four years

Abstract

This article analyzes the organization and initial performance of the emergency psychological assistance (EPA) service for the Volgograd Region population, which has been operating within the 112 Emergency Call Department since 2021. Purpose: Detailed description and evaluation of the service's performance during the first four years of operation. The research objective is to analyze the dynamics of demand for EPA, identify the population's key needs in crisis situations, and evaluate the 112 service's contribution to ensuring the region's psychological safety. Materials and methods: This study examines residents' primary requests received by the 112 service and analyzes the forms and methods of providing EPA, including remote counseling and coordination with other services. The study utilizes the psychological service's accounting and reporting documentation, which describes all calls to the service during the specified period. The research method utilized content analysis of the data to assess the subject matter, geography, and quality of the assistance provided. Results and discussion: In four years, dispatch psychologists handled 1 855 calls and provided assistance in 1 526 cases. The study's scientific contribution lies in systematizing the experience of organizing and providing emergency psychological support through the "112" service in the region and identifying key population needs for psychological support in crisis situations. The scope of application includes optimizing the operation of emergency psychological support services and developing initial training programs for dispatch psychologists. Conclusions: The implementation of emergency psychological support through the "112" service at the State Public Institution of the Volgograd Region "Control and Communications Center" in the Volgograd Region demonstrates the high demand and effectiveness of remote psychological support for the population. This initiative ensures prompt coordination between emergency services and psychologists, promptly responding to residents' current and traditional crisis needs, including post-COVID conditions, phobias, stress related to the current geopolitical situation, and suicidal tendencies.

Organizational structure and analysis of the Regional Telemedical Center based on the Volgograd Regional Clinical Hospital

Abstract

The issues of analyzing the activities of regional telemedicine centers are currently relevant and are due to the development of modern research methods and the development of telecommunication technologies. Purpose: To determine the structure, functional features, and target audience of the Regional Telemedicine Center and to analyze its activities at the regional level. Materials and methods: The retrospective analysis of the activities of the Regional Telemedicine Center of the Volgograd Regional Clinical Hospital No. 1 was conducted based on the center's analytical report and data from the interactive dashboard for 2024 and 2025. We analyzed the number, type, forms, and profiles of telemedicine consultations. We used descriptive statistical analysis, structural analysis, and visual data analysis. Results and discussion: The results of the study determined the effectiveness of consultations using telecommunication technologies in the "doctor-doctor" format, as well as in the "doctor-patient's legal representative" format. In addition, diagnostic measures and medical and rehabilitation measures were carried out in a remote format. As a result of the analysis of the center's activities in 2024, 4 141 specialized telemedicine consultations were conducted. 2 881 consultations were conducted in the "doctor – patient" format, and 732 consultations were conducted in the "doctor – doctor" format. In addition, 528 consultations were held with the participation of specialists from federal centers. In 2025, the number of consultations increased by 23.9 % and reached 5 132 consultations. There was also an increase in the number of consultations involving leading specialists from federal centers. The Regional Center's work also helps to mitigate the shortage of specialists, including in remote areas of the region. Conclusion: The development of the telemedicine network in the regions requires further development and is closely related to integration into regional information systems, the need for complete patient information and high data protection, and the importance of regular training in digital healthcare for medical and technical specialists at various levels of healthcare organizations.

On the peculiarities of the epidemic process of brutcellesis in the Volgograd Region

Abstract

The article reflects the current state of the problem of brucellosis in the Volgograd region. The increase in the incidence of brucellosis highlights the growing importance of this issue. Purpose: To determine the characteristics of the epidemic process of brucellosis in the Volgograd region and identify the risk factors that caused the epidemiological situation in 2025. Materials and methods: Based on emergency notifications, epidemiological investigation cards and medical histories, we analyzed 35 cases of brucellosis in patients of infectious hospitals in the Volgograd region. The study was conducted with the approval of the Local Ethics Committee. Statistical analysis was carried out using the Microsoft EXCEL 2010 program. Results and discussion: The article presents data on the nature of the epidemic process among the population of the Volgograd Region. The intensity and dynamics of the process are described, and the age and social structure of patients in 2025 are shown. The factors of infection transmission are identified, and the risk factors that caused the epidemiological situation are determined. The connection between animal and human disease is noted, which further highlights the presence of zoonotic foci of infection. The analysis conducted allowed us to formulate practical recommendations and identify preventive measures carried out by veterinary and medical services aimed at reducing the incidence of diseases in the region. As a result of the study, the territories of the region were identified as unfavorable for human brucellosis. Conclusion: Laboratory methods of examination during periodic preventive examinations, regardless of the epizootic situation, increase the detection rate of infected individuals. An analysis of the conditions of infection in patients showed the high importance of the organization of trade in livestock products as a social factor that determines the intensity of the epidemic process. According to the epidemiological investigation, most of the patients were infected through the alimentary route, with 54.3 % of infections transmitted through dairy and meat products that had not undergone veterinary control. In this regard, the interaction between healthcare organizations, Rospotrebnadzor, the veterinary service, and the heads of enterprises and organizations that work with livestock and process livestock products remains relevant.

A model for regional healthcare workforce provision (illustrated by the Stavropol Region)

Abstract

The problem of the shortage of medical personnel in the Stavropol Territory remains an urgent problem, despite the implementation of government support measures. Insufficient provision of health workers is especially acute in primary care and rural areas, representing a significant systemic limitation. An effective solution requires the implementation of comprehensive models that combine organizational, legal and digital mechanisms. Purpose: The creation and implementation of an integrated model for managing medical personnel in the Stavropol Territory using digital solutions and the potential of a medical university, as well as an assessment of its impact on staffing. Materials and methods: The study was conducted in 2021–2025 on the basis of the federal state budgetary educational institution of higher education "Stavropol State Medical University" of the Ministry of Health of the Russian Federation (hereinafter – STMU). Analytical, statistical methods, sociological survey (488 students, 854 residents, 28 experts) and organizational experiment were used. Statistical processing was performed in IBM SPSS 26.0 with the calculation of the arithmetic mean (M), standard deviation (SD), Kendall concordance coefficient (W); differences were considered significant at p < 0.05. Results and discussion: A multi-level staffing model has been developed and implemented. For 2021-2025 the provision of doctors increased by 11.6 % (p < 0.05), the share of residents employed in the region increased by 24.3 %, and those who left for other regions decreased from 30.7 to 25.2 %. Conclusions: The proposed model has proven to be effective. A key success factor is the integration of medical university capacity and the regional health system. The model can be scaled in other constituent entities of the Russian Federation.

Dynamics of ophthalmological care indicators for the main socially significant diseases in the Saratov Region in the conditions of the federal project "Development of the NMIC Network"

Abstract

Ophthalmic disorders remain one of the leading causes of temporary and permanent disability, as well as reduced quality of life, which determines the high socio-economic importance of the effectiveness of specialized care. The implementation of the federal project "Development of the National Medical Research Centers Network" (2021–2024) entails a substantial transformation of regional healthcare systems; however, the actual impact of new organizational approaches and centers of excellence on ophthalmic services in the regions of the Russian Federation has not been sufficiently studied. Saratov Region is among the territories with a pronounced need to improve access to high-technology ophthalmic care and to modernize infrastructure, which makes the analysis of ongoing changes particularly relevant. Purpose: The aim of the study was to assess the impact of the federal project "Development of the National Medical Research Centers Network" and the organizational and methodological guidance provided by the Volgograd Branch of the S. Fyodorov Eye Microsurgery Federal State Institution on the development of the ophthalmic service in Saratov Region in 2021–2024. Materials and methods: A retrospective descriptive and analytical study was conducted using official medical statistics for Saratov Region for 2021–2024. Data on human resource provision, infrastructure, volumes of high-technology medical care, and the implementation of innovative technologies were analyzed. Results und discussion: The study demonstrated a positive impact of the organizational and methodological guidance of the Volgograd Branch of the S. Fyodorov Eye Microsurgery Federal State Institution on the development of the regional ophthalmic service. An increase in the availability of specialized care, the introduction of modern diagnostic and treatment methods, and the strengthening of human resource capacity were established. Conclusions: The experience of the region can serve as a model for organizing ophthalmic care within the framework of the national project "Healthcare".

Analysis of morphometric parameters of the incisitive foramen based on cone-beam computed tomography data for individual anesthesia planning in the area of the naso-palatiate canal

Abstract

High variability of the anatomical structure of the incisive canal and incisive foramen requires detailed preoperative planning for safe performance of surgical interventions and conduction anesthesia in the anterior maxilla. Purpose: To evaluate the morphometric parameters of the incisive foramen based on cone-beam computed tomography (CBCT) data and to develop a method for digital planning of individual navigation templates for incisive anesthesia. Materials and methods: A retrospective analysis of CBCT scans of 160 patients (80 men, 80 women) aged 25–55 years was conducted. The shape, longitudinal and transverse dimensions of the incisive foramen, and the incidence of accessory incisive foramina were determined. At the prospective stage, intraoral scanning was performed in 30 patients, CBCT data were combined with digital models in RealGide 5, and navigation templates were designed and manufactured using 3D printing. Results and discussion: The shape of the incisive foramen was round or oval in 85 % of cases, and slit-shaped in 15 %. The average longitudinal size was (3.45 ± 1.15) mm, the transverse size was (3.58 ± 1.07) mm. Accessory incisive foramina were detected in 22 % of cases (from 1 to 3). Statistically significant gender differences were established: oval (62.3 %; p = 0.001) and round (60.0 %; p = 0.003) shapes were more common in men, while heart-shaped (74.4 %; p = 0.002) ones were more common in women. The incisive foramen dimensions in women [longitudinal (3.65 ± 0.65) mm, transverse (3.90 ± 0.96) mm] were significantly larger than in men [(3.00 ± 0.44) mm and (3.15 ± 0.75) mm, respectively; p < 0.05]. Based on the obtained data, 30 individual navigation templates for incisive anesthesia were developed and manufactured. Conclusion: The proposed method of digital planning and 3D printing of personalized navigation templates for the first time takes into account the individual anatomical features of the incisive foramen based on CBCT data, which allows to control the trajectory and depth of needle insertion and minimize the risk of complications (bleeding, paresthesia).

The role of myography in quality control of treatment with fixed orthopedic prostheses using a digital protocol

Abstract

A balanced biomechanical complex, which includes elements such as the dentition, the masticatory muscles that raise and lower the mandible, and the temporomandibular joint, ensures efficient and harmonious function of the human masticatory apparatus. The center of mechanical force generation is the masticatory muscles, which are under the control and regulation of the nervous system. Disruption of the geometric shape of the teeth and dental arch causes changes in the uniformity of contractions, rhythm, force, and direction of impact in the muscles that elevate the mandible, leading to overload and underload of the elements of the masticatory apparatus. Purpose: Comparison of the bioelectric activity of the chewing muscles in patients with unilateral defects in the dynamics of treatment with prostheses made using different methods. Materials and methods: 31 people (21 women and 10 men) aged 35 to 50 years with unilateral defects of the dental arches were examined. The examination included individuals with clinically healthy periodontium and no concomitant pathologies. The subjects were divided into two groups based on the method of prosthetics. 1 group: digital method of prosthetics with fixed structures (16 people). 2 group: analog method of making fixed structures (15 people). The study was conducted in 3 stages: before prosthetics, after prosthetics, 21 days after prosthetics. Results and discussion: Studying adaptation to prostheses manufactured using a digital protocol, the results showed a gentle and smooth adaptation of the masticatory muscles to the newly created occlusal contacts, which in most cases were obtained using prototypes of future prostheses. This fact allows for the creation of an optimal occlusal surface corresponding to the antagonistic groups. The use of prototypes allows for a gentle adaptation without causing stress spikes in electrical excitability when new occlusal contacts appear. A significant role in this process is played by the minimal error of this technique, which allows predicting the density of contact of the dentitions in occlusions that constitute the masticatory cycle. This approach allows the muscles to enter the adaptation of the masticatory process gently and safely adapt the elements of the TMJ to the newly created contacts. Conclusion: Restoration of dentition defects with fixed prostheses increases the synchronization of muscle function, and the digital protocol is the optimal method for the most effective patient rehabilitation.

Radiographic variants of the dome of the hard palate and the location of the spinal line

Abstract

The study of the features of anatomical variants of bone structures on a telerentgenogram in lateral projection and the improvement of the research method are relevant for morphology and clinical dentistry. Purpose: Determination of anatomical variants of the dome of the hard palate on telerentgenograms in lateral projection and cone-beam tomograms and assessment of the location of the spinal plane for the orthodontics clinic. Materials and methods: The analysis of 65 telerentgenograms of people with optimal bite ratios of antagonist teeth was carried out. The X-ray images indicated the point of the anterior nasal spine (SNA) and posterior nasal spine (SNP), through which the spinal line (plane) was drawn. A reference point was set at the points of the greatest convexity or concavity of the hard palate, which served as a guideline for determining the typological variant of the hard palate. Results and discussion: As a result of analytical work, it was found that the most common variants of the dome of the hard sky was the flat shape, which was found in (49.23 ± 6.2) % of the studied radiographs. The convex shape of the hard palate was detected in (35.38 ± 5.93) %, and the concave shape in (15.38 ± 4.47) % of cases. The differences in the shape of the dome of the solid sky were determined by the deviation of the lines drawn through the dorsal points and through the dome of the sky in relation to the Dreyfus line. With a convex shape of the hard palate, the angle drawn from the anterior spinal point through the bulge of the hard palate is significantly less than the angle drawn through the anterior and posterior spinal points. Conclusion: In the course of the study, it was found that in the orthodontics clinic, when analyzing the position of the spinal plane, drawing it from the anterior spinal point through the point of the greatest convexity of the hard palate is not an objective method of analysis, unlike the method of drawing the spinal line through spinal point landmarks.

Difficulties in diagnosing multiple myeloma against the background of degenerative-dystrophic diseases of the spine

Abstract

Patients with spinal osteochondrosis typically develop persistent chronic pain. However, this typical clinical manifestation of degenerative-dystrophic changes in the intervertebral discs associated with osteochondrosis may conceal prognostically much more serious diseases (including malignant neoplasms), especially in the absence of the classic radiographic symptom of "punched-out osteolysis foci (punches)" characteristic of myeloma. This is also facilitated by the fact that most patients with malignant neoplasms can maintain relatively satisfactory general health for a long time, even despite the onset of pain. Materials and Methods: This study analyzed the diagnostic search in a 65-year-old patient complaining of lower back pain, using data from outpatient and inpatient medical records. Results and discussion: This clinical case demonstrates the insidious nature of multiple myeloma, which can be masked not only by other, prognostically more favorable conditions, but also by malignant neoplasms. It demonstrates that targeted, persistent diagnostics enables timely detection of the disease, which sometimes hides behind the clinical veil of classic degenerative-dystrophic changes in the spine, as well as in the form of metastatic lesions. Conclusions: This clinical case report demonstrates the similarity between the typical clinical presentation of chronic low back pain exacerbation associated with degenerative spinal diseases and the onset of multiple myeloma (MM), characterized by multiple foci of osteodestruction. Given that such patients may receive prolonged treatment for radiculopathy from a neurologist, the crucial time to initiate treatment for multiple myeloma may be missed.

Assessment of cardiovascular risk factors, endothelial dysfunction biomarkers, and proinflammatory cytokines in patients with rheumatoid arthritis

Abstract

Cardiovascular diseases (CVDs) remain the leading cause of mortality and disability. Immune-inflammatory rheumatic diseases, such as rheumatoid arthritis (RA), significantly increase CVR. Purpose: In an open comparative observational study, to analyze traditional and disease-associated cardiovascular risk factors in patients with RA and arterial hypertension (AH), and to determine the level of circulating markers of systemic inflammation and endothelial dysfunction. Materials and methods: The study included 150 patients with established RA (ACR/EULAR criteria, 2010) and 50 individuals with hypertension. In addition to the standard examination stipulated by current clinical guidelines for RA and hypertension, the presence and severity of joint syndrome, the presence and type of concomitant cardiovascular pathology were taken into account in patients with RA. Laboratory tests included acute-phase reactants, immunological markers of RA, and specific laboratory parameters: quantitative levels of tumor necrosis factor-α, interleukin-1β, interleukin-6, endothelin-1, and von Willebrand factor in the patients' blood. Statistical processing of the results was performed using parametric analysis methods with normal data distribution. Results and discussion: Patients in the study (RA) and control (AG) groups did not differ significantly in demographic and anthropometric parameters. More than half of the patients with RA (62.67 %) fell into the very high or high CV risk category. A comparison of the frequency of traditional risk factors in the study groups revealed that patients with RA had higher systolic blood pressure, higher mean total cholesterol levels, and less frequent use of antihypertensive therapy. Disease-associated cardiovascular risk factors were present in the vast majority of patients with RA and were more frequently recorded in individuals with high and moderate RA activity. Atherosclerotic CVD was represented by various forms of IHD, and CHF was present in every fifth patient with RA. Patients with RA had a significant increase in the levels of soluble markers of endothelial dysfunction – endothelin-1 and von Willebrand factor, as well as a high level of circulating pro-inflammatory cytokines – TNF-alpha, IL-1, and IL-6. Conclusions: Thus, the vast majority of patients with RA have comorbid cardiovascular disease, which has demonstrated a direct association with RA disease activity. A significant increase in key proinflammatory cytokines, combined with elevated levels of endothelin-1 and von Willebrand factor, was detected in the blood of patients with RA, confirming the role of systemic inflammation in the development of vascular lesions in RA.

Results of using alphacyanocrylic acid in allohernioplasty of postoperative ventral hernias

Abstract

Prevention of postoperative seromas in the treatment of abdominal wall hernias always remains a major concern in herniology. Seromas, as a complication following hernial orifice repair, are a leading cause. When dividing the anterior abdominal wall tissues, especially with extensive separation, numerous blood and lymphatic vessels are exposed, which predicts the development of clinically significant postoperative seromas in the surgical area. This problem remains unresolved, prompting surgeons to actively seek preventative methods for postoperative serocomplication prevention. In our opinion, the way to prevent the formation of seromas is to use a medical adhesive based on alpha-cyanocrylic acid (Sulfacrylate). Taking into account its properties – rapid polymerization, hemostatic and lymphostatic, as well as bactericidal effects, the ability to biodegrade in the anterior abdominal wall (fragmentation and resorption) subsequently, the percentage of postoperative seromas significantly decreases. Purpose: To determine the effect of using sulfacrylate medical glue as a preventive measure for clinically significant postoperative seromas in allogernioplasty of incisional ventral hernias. Materials and methods: The study included 126 patients with postoperative ventral hernias of various sizes, including 50 (the study group) who underwent allohernioplasty with the additional use of sulfacrylate adhesive composition, and 76 (the control group) who did not use glue. Results and discussion: The use of the adhesive composition (sulfacrylate) demonstrated the ability not only to additionally fix the mesh implant but also to significantly reduce the percentage of clinically significant seromas in the surgical area. Conclusions: The use of medical acrylic glue is accompanied by a statistically significant decrease in the formation of clinically significant seromas in the supraventricular space during surgical treatment of postoperative ventral hernias.

The effect of consumption of non-alcoholic tonic drinks on the performance of military sports training of young men of military age

Abstract

The consequences of the consumption of non-alcoholic tonic drinks (BTN) by young men of military age remains insufficiently studied today. Purpose: To study the effect of BTN on the functional state of the body. Additionally, the effect of BTN on the eating behavior of conscripts was studied. Materials and methods: The impact of BTN consumption on the indicators of military sports performance was assessed on the basis of data from military-patriotic competitions. Medical documentation and protocols with the results of compliance with the standards have been studied and processed. The subjects were divided into the main and comparative groups according to the principle of BTN consumption. The following were evaluated: strength, agility, speed, endurance, attention and accuracy. The clinical and functional parameters of the body were also evaluated. Results and discussion: An assertion has been made about the relationship between the duration of BTN consumption and the frequency and volume of consumption per day. A change in eating behavior was noted. The lack of energy intake from food and energy expenditure in the main group and its effect on exercise performance were determined. Energy deficiency due to the lack of a morning meal due to the lack of BTN consumption, a meal was suggested. The alleged influence of eating disorders on the performance in meeting the standards has been revealed. Conclusion: The complex negative impact of BTN on the eating behavior and functional state of the conscripts indicates the need for further research.
